05Acknowledgments and submission

I confirm that I am the applicant’s parent or legal guardian, have reviewed this application, and consent to the applicant’s participation in the selection process. * I confirm that the information provided is accurate to the best of my knowledge. * I understand that authorized Foundation selection committee members will review my information to evaluate this application and administer the scholarship. * I understand that one recipient demonstrating financial need will receive reimbursement of up to $500 for documented club soccer fees or soccer equipment. If selected, I must provide receipts before payment. Payment cannot exceed the eligible expenses documented. A parent or guardian must handle reimbursement for a recipient under 18. *
